I, the Parent/Guardian of the player's name hereon acknowledge the participation of their

son/daughter in the Mission Viejo Football Club (MVFC) Mini Kickers camp. As in any event

related to sports this may result in injury. The undersigned parent/guardian therefore

releases MVFC, Its Agents, Officers, Coaches and Players, from all Liability or

Responsibility for any claim, damage or legal action on behalf of the player, parents, heirs

or personal representatives, arising from any injury the player may sustain while

participating in this soccer related event.
